Puget Sound Chapter CSI Student Sponsorship Application Form

Applications are only to be sent between September 1 and November 11. Fill out this online form for consideration of a Puget Sound Chapter CSI student sponsorship. Sponsorships include membership in CSI and the Puget Sound Chapter as well as no-cost attendance to the monthly meetings. Please fill out the following and click on "submit" when complete.
